New Patient - After you have scheduled your first appointment with us, we ask that you please arrive for your appointment 15 minutes early to fill out your personal dental forms.  Some of the information that we will be requesting includes: Dental History (recent x-rays or copies from last dental visit are welcome) Medical History (bring list of medications, names of Doctors if extensive) Primary Insurance (bring dental insurance card with you) Additional Insurance (and secondary insurance you may have) HIPAA (signing of privacy act)

We care deeply about your dental health and our support team will make every effort to make your visits as comfortable as possible. We give every patient a comprehensive treatment plan in writing along with our current fees. Our treatment coordinator will work with you on an individual basis to help make treatment affordable to you. In an ongoing effort to provide quality dentistry at an affordable cost to our patients we ask that services be paid for at the time of service. For your convenience we accept cash, check, Master Card, and Visa. Financing options are available for extensive treatment plans.
